What is a Nurse Practitioner?
A Nurse Practitioner is an advanced practice nurse with a Master’s and/or Doctorate degree. They have the knowledge to evaluate patients, order and interpret diagnostic tests, create and manage treatment plans, and prescribe medications—all without physician oversight.
How do I get started?
The first step of becoming a patient with SWGeriatrics is to submit a completed New Patient Packet. You can request our New Patient Packet here. It is important to fill out the forms completely. Blanks or missed information will delay our ability to confirm insurance and delay the admission process.
How long does the approval process take?
After receiving a completed New Patient Packet, we call and verify the insurance coverage so we can inform the patient what, if any, cost will be incurred. Standard turn-a-round is one business day.
How soon can a SWGeriatrics provider see me?
Our providers have flexible schedules, and the patient will be scheduled as quickly as possible. After a new patient is accepted, we’ll communicate with the patient (or responsible party) to schedule the first appointment.
What is the difference between SWGeriatrics Nurse Practitioners and a Home Health Agency?
As primary care providers, we determine the medical necessity for Home Health, contact the Home Health Agency for the patient, and write the orders for the patient to be admitted. We communicate with the Home Health staff and nurses and we write the orders for patient care. When the Home Health Agency discharges the patient, we continue taking care of the patient as their primary care. Our patients do not have to be homebound to receive a visit from us.
Home Health services are temporary, approved by insurance and require the patient be homebound. After receiving an order from us, the agency nurse visits the patient to fill out paperwork to confirm eligibility. They offer physical therapy, occupational therapy, speech therapy and nursing visits.
When should someone consider becoming a patient with SWGeriatrics?
- As a new Tucson resident, or a seasonal vistor that needs a Primary Care Provider
- When traveling to a doctor’s office becomes difficult (temporary or permanent) because of health issues
- When being admitted to an assisted living or adult care home
Do you see patients in an office?
We do not have a physical location. Our providers travel to the patient’s home or place of current residence (i.e., assisted living facility, nursing home) to provide geriatric care.
